Course Description: Case Management involves collaboration with the multi-disciplinary team to look at all aspects of the patient’s care from their clinical picture to their social picture. This program will examine the case manager’s role in providing care in the clinical setting as well as recommendations of appropriate transitional resources.
Target Audience: Social Workers, Nurses, Licensed Psychology Counselors, Medical Assistants, and AHEC.
