Course Description: This program will help healthcare professionals conduct effective transition planning. Participants will learn how to assess when a transition to a higher level of care is necessary and become familiar with the tools and resources available to assist in this process.
Instructional Method: Asynchronous online PowerPoint® lecture, Pre-Test, Post-Test and EvaluationTo pass this course you must view the lecture, complete the pre-test and evaluation and score at least 80% on the post-test.

Target Audience: Nursing, Social Workers, and other interested healthcare providers
Learning Outcomes:At the end of this presentation, the learner will be able to:
Identify the needs of aging Americans
Discuss perspectives on the resources available to older adults
Collect information on how to appropriately assess seniors for transitioning to a higher level of care
Social Workers: This program has been approved for 1.75 clock hours of social work continuing education by Upstate AHEC, as part of the SC AHEC system, under its accreditation by the SC Board of Social Work Examiners.
Licensed Professional Counselors and Therapists: This program has been approved for 1.75 hours of continuing education by SC AHEC under its accreditation by the SC Board of Examiners for Licensure of Professional Counselors, Marriage and Family Therapists and Psycho-Educational Specialists.
Licensed Nurses: Upstate AHEC is an accredited provider of Nursing Professional Development by the American Nurses Credentialing Centers Commission on Accreditation. Upon successful completion of this program, attendees will receive 1.75 contact hours.
